Welcome to the DVT Risk Assessment Tool

This tool provides a simple, convenient way for you to help assess your patients' risk of deep vein thrombosis (DVT) in the form of an easy-to-administer, easy-to-answer questionnaire using 4 sets of statements. Users select statements that apply to accrue their risk score. Each risk category is explained in terms that are easy for patients and their loved ones to understand, and they are reminded that only their doctor can determine their DVT risk. Personalized results can be printed once the Risk Assessment Tool questionnaire is completed.
